How to Register

To register at the practice all you need to do is complete a registration and health form -see the link at the bottom of this page to print a copy or you can obtain the forms directly from Reception.

If you do register on line you will still have to present (personally) at the surgery with two forms of identification, one being confirmation of address and the other being photo ID - passport/drivers licence etc.

Once you are registered you can then book in for a new patient check with one of our Nurses, for an introduction to your new practice. 

We are happy to register people from local areas which are Exwick, Cowley, St. Thomas, St. David’s and Alphington
